We work collaboratively with support coordinators, families, nominees, allied health professionals and other providers. Our goal is to make the referral and intake process clear while keeping the participant’s choice, safety and preferences at the centre.

- Support needs, communication preferences and mobility requirements
- Risk, behaviour support or health information relevant to safe service delivery
- Preferred commencement date and service schedule
- Contact details for the participant's nominee or support coordinator, where applicable
